Dump trucks are used primarily to haul dirt and soil to and from the job site.
Fatalities
347 cases or about 3.2% of recorded incidents are related to dump trucks.
Typical Example
386.Employee#1 was run over by a dump truck while the truck was backing up to unload asphalt for road work. The employee was in the blind spot of the driver’s mirror. The employer’s truck was equipped with a back-up alarm and other employees yelled and blew horns to warn him to get out of the way. He did not pay attention to the warning and was killed.
Avoiding Fatalities
Avoid being in the truck’s blind spot.
Pay close attention when crossing or walking along moving vehicle’s path.
Be aware of overhead power lines.
Use proper support when repairing hydraulic parts on the truck’s bed.
Make sure back-up alarm is in proper working condition.
Use a flagperson.
Common Activities
Work being done outside when thunder is heard in the distance.
Workers on roof tops and up in lifts have increased exposure to being struck.
The “One More” mentality is dangerous.
Construction Welding
Weld - A mix of metals that joins at least two separate parts. Welds can be produced by applying heat or pressure, or both heat and pressure, and they may or may not use an additional filler metal.
Common Welds: arc weld, groove weld, spot weld , groove weld, fillet weld
Welding takes place on a construction site both on the ground level, as well as high in the air on multistory buildings. Welding will often take place in confined spaces.
Falls
Employee #1 was welding metal decking to the joists adjacent to the decking edge when he fell backward and dropped 15 ft onto the concrete floor. He sustained head injuries and was killed. The decking was not equipped with a standard guardrail and no personal fall protection was being used.
