DEFINITION
Chronic infectious communicable disease caused by Myco.leprae, mainly characterized by lesions in skin and/or polyneuritic changes due to involvement of and damage to the peripheral nerve trunks.
Hansen disease is caused by a bacillus, Mycobacterium leprae, first described by G. Amauer Hansen, MD, of Norway in 1874, which multiplies very slowly and mainly affects the skin, nerves, and mucous membranes.
Incubation in humans ranges from 6 months to 8 years, and the bacilli are believed to enter the body through the skin or mucous membranes of the upper respiratory tract. The extent of involvement and the type of manifestation after infection depend on the immune response of the host and may consist of complete resistance, no disease, incomplete resistance, and the indeterminate, which can progress to the other forms
OCULAR MANIFESTATIONS
A . Indirect damage due to nerve involvement
Motor nerve supply to muscles of lids (temporal & zygomatic brs. Of 7th cranial nerve)
Sensory nerves supplying cornea & conjunctiva (anterior ciliary N & lacrimal+ infratrochlear branch of ophthalmic division of Trigeminal nerves)
B . Lesions due to actual involvement of Eye Tissues
Direct Spread of leprous process from eyelids or surrounding parts of face/lacrimal passages
Spread of bacilli through blood stream
During a lepra reaction, deeper tissues of eye may be involved with lesions similar to ENL
